I think this is the reason you buy Director's Cuts or extended version. The ending is different and makes you think. In the "Special Features" you get a great "Making Of" and why the original is so different from this version. I think that is why you want to buy this item - the insight and process is great.
I disagree with the director, I think in the end that "Porter"is heading back accross the same bridge to the same hack doctor to save his life again. This time he has the money, the girl and the car - I think that's why Gibson's character is smiling, he got away with it.

I was extremely disappointed with this movie. I did not know or realize that the directors cut was going to be so different from the released version. Hated this ending, it just kind of ends. Get the original movie and you will be happier for it.
